Acitretin (Brand name "Neotigasson/Soriatane") may be a temporary answer?

as you know, hyperkeratosis can occur not only in our hair follicles but in other place too (that are IMO much worse such as palms and soles).
Acitretin is a medicine that is meant to treat even the most severe disorders of keratinization by decreasing the overall keratin production and the differntiation of the keratinocytes (the cells that produce and get filled with keratin).

It has some side effects such as dry lips, eyes etc and basically it's a second generation retinoid after all. However it doesn't have anything to do with the sebacaous (sebum producting) glands so I don't think it may "permenantly make our body drier" such as Isotretinoin as it doesn't affect the sebum production but only the keratin (it is a well known medicine for harsh psoriasis). The worst it can do is damage the liver but I believe we can work around it somehow.. or just taking it once in a while in small doses and maybe it will help a bit.



On another note, have anyone tried Salicylic Acid? it is said to be even more effective than glycolic (AHA) as it is fat-solube and it exfoliates from the most upper layer of the skin thoroughly(I've read that for instance 3% salicylic exfoliates equally to 10% glycolic) and also less irritating.. I've got my pharmacy to prepare me some 10% Salicylic acid with the rest being Paraffin Oil (which sucks but they've got nothing else to put instead execpt vaseline which is worse). Anyway, my next step is to mix Salicylic with Lactic acid, Sulfur and urea and by that creating the ultimate very-severe-KP treatment ! moooowahahahahaha!

That's a very interesting suggestion! Acitretin looks like it's designed for psoriasis, but a few Google hits suggest some may have/be using it for off-label treatment like KP.

I was treated with Isotretinoin (accutane) last year for 5 months, which cured my acne and oily skin, but had no noticeable impact on my extensive KP. I've also tried Adapalene (a topical retinoid), which was only as effective as AHA for me. Hopefully Acitretin would work differently from these two as you say.

I have tried and still use salicylic acid, the beta hydroxy, in the form of Clean & Clear (available in 0.5% and 2%) or Clearasil Cleansing pads (2%). It compliments my daily 15% AHA treatment, the only thing that delivers any results for me besides tanning. Using general purpose moisturizers (especially lactic acid and urea) have only made my KP worse, but I'd be interested to hear how the 10% salicylic acid works for you.

I also wonder if the Clean & Clear 2% solution could be easily concentrated, since it uses an alcohol base which evaporates readily.

Adapelene was good for you? how effective was it???was it as good as AHA 15%?

anyway I've to both the dermatologists in my area and none of them wanted to prescribe me Neotigason... however one of them is still convincible (with a few bucks under da table :P:P:P)
but I don't wanna try it yet, it gots loads of side effects, so i'm gonna try salicylic for a month and if it wouldn't yield any results than i'll go on the Neotigason..

*However the neotigasson has to work, cause it mainly lowers the keratin production in our skin by decreasing keratinocytes differentiation, and if you xfoliate daily + using acitretin for lets say , two months, then you can achieve smooth skin! and than you can go to lower dosages of neotigason / stop it and continue exfoliating daily every other day with some acid.

I wouldn't say Adapelene was "good". It was an off-label use of an expensive prescription, and made sensitive skin (like the back of my knees) red, even though I tried to avoid the area.

It worked practically the same as AHA (e.g. the bumps are smoother, there's less inflamation, but the noticeable red and brown dots remain). I wouldn't recommend it over AHA/BHA.

Well Ill keep on trying BHA (Salicylic) and i'll reply every once in a while to tell if i'm getting any results.. right now it's been 3 days since i began using it and nada.

That girl probably took more than she should have.. her doctor told her to go 2 pills every day (and prolly 25-30mg pills = 50-60 mg per day which is ALOT!)

It is clearly told in the instruction to have 0.5mg for every kg you weigh...

And BTW, you can always lower your dosage if you've got worsening in the side effects or temporarily stop taking it.
and for people that have very severe kp almost all over their body (like me), this might be a great start.
